Who we are
Total Care Medical Specialists, LLC (Total Care Medical Specialists) is the medical practice of John Beelitz, M.D., at 3365 Burns Road, Suite 203, Palm Beach Gardens, FL 33410. You can reach us at (561) 203-8991.
What this page collects
When you send the request form, we receive:
- your name and mobile number, and your email and best time to call if you give them;
- which of the two call and text boxes you checked, when you checked them, and the version of the wording you saw;
- the address of this page and the page that sent you here;
- if you came from an advertisement, the campaign tags in that link and whether the link came from a Meta (Facebook or Instagram) or Google ad.
Please do not enter medical details in the form. We ask only for what we need to call you back.
What stays on your device
If you arrive from a link with campaign tags, the page keeps those tags in your browser's session storage until you close the tab, so they can be attached to a request you choose to send.
No advertising trackers
This page runs no advertising pixel, no analytics tool and no advertising or analytics cookies. If that ever changes, we will update this policy first and name the tool here.
How we use your information
- To call you back about your request and arrange a consultation.
- If you checked the first box: to call and text you about your request and scheduling, appointment reminders and office follow-up, which may be sent with an automated system.
- If you checked the second box: to send marketing calls and texts about our services, updates and educational information.
- To keep a record of your request and of the consent you gave, and to protect this page from abuse.
Who receives it
We do not sell or rent your personal information, and we do not share it with anyone for their own marketing. It is handled only by:
- Total Care Medical Specialists, to respond to you;
- HighLevel (LeadConnector), the contact and messaging system where your request is stored and from which our calls and texts are sent;
- Vercel, which hosts this page and passes your request to that system; it processes your IP address and browser details to deliver the page, limit repeated submissions and keep short-lived security logs;
- RBV Media, the agency that manages this page and the contact system for the practice.
We may also disclose information when the law requires it.
No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
Calls and texts
Message frequency varies. Message and data rates may apply. Reply STOP to any program text to unsubscribe or text HELP for assistance. You can also withdraw consent or request help by calling (561) 203-8991 or telling us in person. Agreeing to calls or texts is never a condition of purchasing anything or of receiving care. Text messages are not secure, so please do not send medical details by text. Our Terms describe the call and text program.
Your health information
If you become a patient, our Notice of Privacy Practices explains how we use and protect your health information. It is available at the office or by request.
How long we keep it
We keep your request for as long as we need it to respond to you, to keep records the law requires, and to resolve any dispute. You can ask us to delete it at any time, except where the law requires us to keep it.
Your choices
Call (561) 203-8991 to ask what we hold about you, to correct it, to ask us to delete it, or to tell us you no longer want calls or texts.
Security
This page is served over an encrypted connection, and your request goes only to the system named above. No method of sending information over the internet is perfect, which is one more reason to leave medical details out of the form.
Children
This page is meant for adults. We do not knowingly collect information from anyone under 18.
Changes
If we change this policy, we will post the new version here with a new date.
